International Consignment Note for Road Freight Transport

The CMR form is an essential document attached to the transport contract in international road freight transport. It serves as an official consignment note containing all key information required for shipment handling and cross-border logistics control.
This product is a 5-part numbered carbon copy set including three original copies – red for the sender, blue for the carrier and green for the recipient. Two additional black copies are intended for other carriers, customs authorities or administrative purposes. The form provides sufficient space for all mandatory transport details. After separation, each sheet measures 21 × 30 cm, corresponding to A4 format. The form is printed in Czech-German version with English and French translation on the reverse side.
